Welcome to the Elms Creatives family!
Thank you for choosing Elms Creatives. We are delighted to welcome you and your family to our growing performing arts community.
If you have registered your child for classes with us, please complete the Elms Creatives Family Enrolment Pack below before your child's first class.
The Enrolment Pack contains the information and agreements we need to complete your child's enrolment and help us provide a safe, supportive and enjoyable experience for every student.
Please note: Our Junior classes are currently the only classes available for enrolment, for children aged 6-11 years. Please join our mailing list by clicking here to stay up to date with upcoming classes.
What do I need to do?
1. Download and Read the Enrolment Documents

Please read and follow all steps carefully.
The enrolment pack is a fillable PDF, meaning you can complete it electronically using a computer, tablet or compatible device.
Before completing the Family Enrolment Pack, please also download and carefully read the Elms Creatives Parent Handbook.
The Parent Handbook contains important imformation about our classes, expectations, policies, procedures and what you can expect from Elms Creatives. By signing the Enrolment Form, you confirm that you have read and understood the Parent Handbook and agree to the information and expectations outlined within it.
Please make sure you have read both documents fully before completing and signing your enrolment form.
2. Complete the Enrolment Pack
Please complete all sections that apply to your family.

If You Are Enrolling More Than One Child
You only need to complete the Family Enrolment Pack once.
Please complete an Additional Child/Sibling Registration Form for each additional child joining Elms Creatives.
Medical, emergency and additional support information should be completed separately for each child where applicable.
3. Sign and Save Your Completed Pack
Once you have completed the form, please make sure that all relevant sections have been completed and that the required declarations, consent and signatures have been provided.
Please save the completed PDF before returning it to us.
We recommend saving the file using your child's surname and first name, for example:
Smith-Sophie Enrolment Pack.pdf
4. Return Your Completed Enrolment Pack
Once completed, please return your Enrolment Pack to us by email.
Please attach your completed PDF to the email and
send it to:
info@elmscreatives.co.uk
If you are enrolling more than one child and have completed additional child forms, please include these with your email.
Please do not send the completed form until you have finished all relevant sections.
When should i return it?
If you have registered your child for September classes, please return your completed Enrolment Pack before your child's first class.
This gives us time to review the information provided and ensure that we have everything we need before your child joins us.
